Job summary

Bloom Healthcare is seeking an NP or PA to deliver primary care in home and community settings across Dallas/Fort Worth. You’ll manage a defined patient panel, address complex medical needs, and collaborate with an interdisciplinary team using Bloom’s data systems to improve outcomes.

Qualifications

• Master's degree and active Missouri licensure.

Responsibilities

• Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from the clinical operations, triage team, and MD leadership.
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
• Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
• Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
• Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
• Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
