Job summary

Bloom Healthcare in San Antonio is seeking a Primary Care NP/PA to deliver relationship-based, home- and community-based primary care for medically complex seniors. You’ll work with an interdisciplinary team and advanced data systems to manage chronic and acute conditions, address social determinants of health, and provide comprehensive evaluations.

Qualifications

• Master's degree in a nursing or physician assistant program with active license.

Responsibilities

• Serve as the primary care provider for a defined patient panel, delivering longitudinal care in patients' homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
• Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
• Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
• Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits, and drive outcomes.
• Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.

What You’ll Do

• Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
• Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
• Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
• Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
• Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
