Job summary

Physician Care Centers (PCC) seeks a Nurse Practitioner – Primary Care in Lake Mary, FL. You will provide comprehensive, patient-centered care across the lifespan, collaborating with physicians and care teams to diagnose, treat, and prevent conditions in a high-quality, efficient setting.

Responsibilities include exams, testing, treatment planning, prescribing, preventive care, and chronic disease management, with emphasis on patient education and team collaboration.

Qualifications

• Master's or Doctorate in Nursing (MSN or DNP).
• Graduate of an accredited Nurse Practitioner program.
• Current RN license and NP license in practicing state.

Responsibilities

• Perform comprehensive physical examinations and health assessments.
• Diagnose and manage acute and chronic illnesses.
• Order, interpret, and evaluate diagnostic tests including labs and imaging.
• Develop and implement individualized treatment plans.
• Prescribe medications in accordance with state regulations.
• Provide preventive care, immunizations and screenings.
• Coordinate care with specialists and other healthcare professionals.
• Document encounters accurately in the EHR and participate in quality initiatives.
