Job summary

TEEMA is seeking a licensed NP or PA to provide comprehensive primary care in a Minnesota Health Center. The APP will deliver preventive and acute care for patients aged 2 and older, including well-woman and well-child visits, immunizations, and care coordination with a multidisciplinary team.

This role requires strong clinical assessment, excellent communication skills, and the ability to manage routine primary care and urgent visits independently within a collaborative setting.

Qualifications

• Licensed NP or PA with active Minnesota licensure.
• Experience providing care to pediatric and adult patients in a primary care setting.
• Epic EMR experience preferred.
• Strong clinical assessment, diagnostic and patient education skills.

Responsibilities

• Provide comprehensive primary and preventive care for patients aged 2 and older.
• Conduct annual physicals and preventive health examinations.
• Manage acute and same-day care for illnesses and minor injuries.
• Perform well-woman exams and well-child visits.
• Administer immunizations and coordinate care with other providers.
• Document encounters in the EMR accurately.
