Job summary

Community Health Centers seeks a Nurse Practitioner to deliver comprehensive ambulatory primary care, including preventive and chronic care, in collaboration with the health care team.

Responsibilities include adherence to guidelines, patient evaluation, referrals, and EHR documentation. Candidates should hold a Master's degree (or post-Master's) and be prepared for cross-site coverage and ongoing professional development.

Qualifications

• Master's degree or post-Master's degree certification required
• Older graduates (before Oct 1, 1998) exempt from above requirement
• Submit supporting documentation showing program compliance with Board guidelines

Responsibilities

• Be familiar with CHC's program philosophies, goals, and objectives; comply with CHC policies, procedures, and regulations.
• Represent CHC values and mission in the community.
• Adhere to evidence-based guidelines, standards of care, and practice.
• Review patient reports (labs, X-rays, EKGs); sign-off and follow up.
• Facilitate referrals with social services in coordination with care team nurses.
• Provide off-hours and Saturday clinic coverage as assigned by Leader/CMO.
• Rotate to other CHC centers as needed for coverage.
• Participate in mandatory CHC meetings and training courses.
