Job summary

U.S. Department of Veterans Affairs’ Boise Health Care System is seeking a Nurse Practitioner (Primary Care) to manage a patient panel within the PACT model.

You will provide comprehensive primary care, perform diagnostics, and coordinate care with the multidisciplinary team in a Monday–Friday schedule. You will work within the VA’s Education Debt Reduction Program framework, with competitive salary, generous leave, and extensive total rewards.

Responsibilities

• Independently manages a patient panel in Primary Care using evidence-based practice.
• Develops and implements individualized care plans for health maintenance and disease management.
• Orders and interprets diagnostic studies; prescribes pharmacologic and non-pharmacologic therapies.
• Provides patient and family education and collaborates with the Primary Care Team.
