Job summary

Monadnock Community Hospital's Jaffrey Family Medicine seeks a compassionate Nurse Practitioner (ARNP) to join our primary care team in Jaffrey, NH. This is a high-quality opportunity to practice independently while collaborating with experienced physicians.

You will conduct histories and exams, diagnose and manage acute and chronic conditions, develop individualized treatment plans, order tests, provide preventive care, and maintain accurate EMR documentation.

Qualifications

• Graduate of an accredited School of Nursing.
• Successful completion of an approved Nurse Practitioner program.
• Current New Hampshire Advanced Registered Nurse Practitioner (ARNP) license.
• Current DEA registration.
• Eligibility for professional liability insurance coverage through MCH.
• One or more years of experience in a clinical or ambulatory practice setting (preferred).

Responsibilities

• Conduct complete patient histories and physical examinations.
• Assess, diagnose, and manage acute and chronic medical conditions.
• Develop individualized treatment plans and coordinate patient care.
• Order and interpret laboratory tests and diagnostic studies.
• Prescribe treatments and monitor patient progress.
• Provide preventive care and patient health education.
• Collaborate with physicians and other healthcare providers to ensure high-quality, coordinated care.
• Make appropriate referrals to specialists and community resources.
• Triaging patient calls and providing clinical consultation when appropriate.
• Monitor and manage patient care between visits.
• Participate in community outreach and health education initiatives.
• Maintain accurate and timely documentation within the EMR.
