Job summary

A healthcare provider in Lafayette, IN is seeking a passionate Primary Care Nurse Practitioner to deliver life-changing care and empower patients. The ideal candidate will have a Master’s degree in Nursing, an active Indiana RN license, and Nurse Practitioner certification. Responsibilities include comprehensive nursing care, conducting physical exams, and creating individualized treatment plans. Qualifications

• Active or eligible Indiana RN license in good standing.
• Nurse Practitioner certification in Family Practice, Psych/Behavioral Health, or Pediatrics.
• Eligibility for active prescriptive authority/DEA licensure.
• Preferably 2+ years of related experience.

• Community health or mental health experience is a plus.
• A valid driver’s license, reliable vehicle, and current auto insurance.

What You Bring
