Qualifications

Under the guidance of the primary care operations team, the Primary Care Nurse collaborates with providers to deliver patient-centered care that is safe, effective, and efficient. The role emphasizes adherence to mission, values, and service standards.

Key responsibilities include managing triage calls/messages, partnering with patients and clinical team members, and driving quality outcomes for an assigned panel.

The position is based at a clinic location with a schedule Monday through Friday, 8:00 am to 4:30 pm, with no nights, weekends, or holidays. After an initial on-site training period, the role offers a 50/50 hybrid work model and requires attendance at monthly on-site meetings.

Qualifications include active LPN or RN licenses and preferred 2-3 years of experience. Skills such as proficiency with Epic EMR, patient triage, computer competency, excellent time management, and adaptability are highly valued.
