Job summary

QuadMed in Peru, IN is seeking a Nurse Practitioner or Physician Assistant to join our outpatient primary care team in the North Central and East Central Consortium. The role emphasizes preventive care and building meaningful relationships with patients, with a 40-hour work week.

The position requires a Master’s degree in an NP or PA program, national board certification, active licensure, and proficiency with EPIC EMR. DOT certification may be needed depending on duties and location.

Qualifications

• Master’s degree in NP or PA program from an accredited college/university.
• Minimum three (3) years primary care practice experience.
• Ability to work independently.
• Experience with an Electronic Medical Record (EMR).
• Active and unrestricted applicable licensure.
• Current CPR (BLS) certification or must be obtained within 60 days of hire.
• NPI & DEA.
• Current National Board Certification.
• May be required to obtain and maintain a valid DOT Medical Examiner Certification depending on responsibilities.
