Job summary

Harbor Health in San Antonio’s Alamo Heights practice seeks a team-oriented Nurse Practitioner or Physician Assistant to lead our multi-specialty clinic in a primary care setting. You will provide safe, effective, and appropriate services while collaborating with the clinical team to support our payvider model.

You will deliver health maintenance, chronic condition management, and referrals with attention to quality care and documentation in the EMR.

Qualifications

Graduate of an accredited PA or NP program with national certification.
Current Texas license to practice as NP/PA.
Certified in Family Medicine practice or eligible.
Current DEA and CPR certification.
Valid Texas driver's license and driving record acceptable.
Able to meet credentialing and privileging requirements.
Comfortable working as a solo provider within a clinic team.
Proficient with electronic health records and strong communication.
Bilingual in English/Spanish preferred.

Responsibilities

Interview members, take medical history, perform exams, order tests, and prescribe treatments.
Provide continuity in managed care for patients with chronic conditions.
Refer to secondary/tertiary care with the care team collaboration.
Perform procedures per issued privileges and health maintenance visits.
Conduct immunization reviews and health education; perform employment exams.
Document encounters accurately in the EMR and participate in care team meetings.
Contribute to quality review, clinical committees, and educational initiatives.
