You will provide preventive and chronic care, order tests, educate patients, and collaborate with a care team to deliver high-quality outcomes.

Qualifications

• Education: completed NP or PA program and licensure in state of practice.
• Clinical experience: minimum of one year in primary care.
• Skills: strong clinical judgment, communication, and collaborative care with providers.

Responsibilities

• Provide comprehensive primary care services including preventive and chronic disease management.
• Perform patient evaluations, exams, and diagnostic assessments.
• Develop and implement individualized treatment plans.
• Order, review, and interpret tests and labs.
• Educate patients on preventive health and disease management.
• Collaborate with physicians and care team to coordinate patient care.
• Maintain accurate documentation in the electronic health record.
