One Medical in Sunnyvale, CA is seeking a licensed NP or PA to join our in-office and tele-health primary care team. You’ll manage a patient panel with acute, chronic and preventive visits and collaborate with both in-house and virtual care teammates.

The role requires an accredited NP/PA program with national certification and California licensure by start date; 2 years outpatient primary care (or 1 year plus fellowship or 1+ year urgent care) in the past five years.

Formación

• Completed an accredited NP or PA program with a national certification
• In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in outpatient primary care, OR 1 year with a 1-year fellowship or 1+ year in urgent care
• State licensed in California, obtained by your One Medical start date

Responsabilidades

• Managing a patient panel with a broad array of patient needs; conducting a mix of acute, chronic, and well visits
• Treating patients in-office or in testing centers as well as conducting occasional tele-health visits
• Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams
• Utilization of your specific clinical training and opportunities to perform in-office procedures
• Willing to obtain additional state licensure and credentialing for One Medical virtual primary care in additional states
