Rockwood Clinic in Spokane, WA is seeking a Nurse Practitioner or Physician Assistant for a Primary Care Team position. The role involves comprehensive primary care with focus on prevention, chronic disease management and collaboration within a multidisciplinary team.

Qualifications

• Minimum of two years' experience as a primary care provider preferred.
• Active Washington State NP or PA license.
• DEA and state prescriptive authority.
• Current BLS for Healthcare Providers certification by the American Heart Association.

Responsibilities

• Provide comprehensive primary care services focusing on prevention and chronic disease management.
• Collaborate with physicians, RNs, pharmacists, social workers and other professionals.
• Document patient encounters accurately and participate in care planning.
• Coordinate transitions of care and support population-based care initiatives.
• Engage in quality improvement to improve health outcomes and reduce costs.
