Qualifications

• Active Washington NP or PA license with DEA and prescriptive authority.
• BLS for Healthcare Providers required.
• Experience in primary care preferred.

Responsibilities

• Provide comprehensive primary care across lifespan.
• Collaborate with physicians, RNs, pharmacists and social workers.
• Manage population health and chronic disease plans.
• Document care accurately and participate in after-hours coverage.
• Support transitions of care and reduce readmissions.
