Job summary

Prosperity Placement Solutions seeks a Nurse Practitioner (NP) or Physician Assistant (PA) to join its primary care team in Dunnellon, Florida. This full-time outpatient role offers autonomy, continuity of care, and the chance to build long-term patient relationships while improving health outcomes.

Responsibilities include evaluating acute and chronic conditions, preventive care, and care planning within a value-based model.

Qualifications

• Licensed NP or PA in Florida with board certification.
• Master's/DNP or PA degree from accredited program.
• Current BLS; ACLS preferred.
• 2+ years in primary care or family/internal medicine.
• Experience with preventive medicine and chronic disease management.
• Ability to work in a collaborative, interdisciplinary team.

Responsibilities

• Provide primary care evaluations, histories, exams, and treatments.
• Manage acute and chronic conditions; develop treatment plans.
• Order/interpret labs, imaging, EKGs, and point-of-care tests.
• Perform office-based procedures within scope of practice.
• Conduct wellness visits and preventive health assessments.
• Coordinate referrals and care with physicians and team.
• Educate patients on diagnoses, medications, nutrition, and exercise.
• Maintain accurate EHR documentation per standards.
