Position Overview

A well-established, physician-led healthcare organization is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its primary care team in Dunnellon, Florida. This opportunity offers a patient-centered outpatient environment, clinical autonomy, and the ability to build meaningful, long-term relationships with patients while improving health outcomes.

The provider will evaluate and manage acute and chronic medical conditions, deliver preventive care, develop individualized treatment plans, and collaborate with physicians and an interdisciplinary care team. The practice follows a value-based care model focused on quality outcomes, preventive medicine, coordinated care, and long-term patient wellness

Key Responsibilities

• Provide comprehensive primary care services, including patient evaluations, medical histories, physical examinations, diagnosis, and treatment of acute and chronic conditions.
• Manage common conditions, including hypertension, diabetes, hyperlipidemia, asthma, COPD, coronary artery disease, obesity, thyroid disorders, depression, anxiety, and musculoskeletal conditions.
• Develop and implement individualized treatment plans involving medication management, preventive care, immunizations, screenings, lifestyle counseling, and chronic disease monitoring.
• Order and interpret laboratory studies, diagnostic imaging, EKGs, point-of-care testing, and other diagnostic evaluations as appropriate.
• Perform office-based procedures within the scope of practice, including minor wound care, incision and drainage, skin lesion treatment, and joint or soft tissue injections, as applicable.
• Conduct annual wellness visits, preventive examinations, health risk assessments, transitional care management, and chronic care management.
• Identify complex medical needs, consult with collaborating physicians when appropriate, and coordinate specialty referrals and ancillary services.
• Collaborate with physicians, nurses, care managers, pharmacists, behavioral health professionals, and other members of the healthcare team to ensure coordinated patient care.
• Educate patients and caregivers on diagnoses, medications, nutrition, exercise, preventive health, and chronic disease management.
• Maintain accurate and timely electronic health record documentation in accordance with clinical standards, regulatory requirements, and payer guidelines.
• Support population health initiatives, quality improvement programs, care gap closure, accurate HCC documentation, and value-based care performance goals.

Qualifications

• Master's or Doctor of Nursing Practice degree from an accredited Nurse Practitioner program, or an appropriate Physician Assistant degree from an accredited program.
• Current, unrestricted Nurse Practitioner or Physician Assistant license in Florida.
• National board certification appropriate to the provider's role and clinical practice.
• Current DEA registration.
• Current Basic Life Support (BLS) certification.
• Advanced Cardiovascular Life Support (ACLS) certification preferred.
• Typically two or more years of advanced practice clinical experience in primary care, family medicine, internal medicine, ambulatory care, or a closely related specialty; primary care experience strongly preferred.
• Strong knowledge of preventive medicine, chronic disease management, evidence-based clinical practice, and population health principles.
• Experience with electronic health records and clinical documentation.
• Excellent clinical judgment, communication, patient education, and interdisciplinary collaboration skills.
• Commitment to compassionate, patient-centered care and improving health outcomes across diverse patient populations.

Compensation:
$160,000 per year
