Bookmark Medical in Brighton, MA is seeking an experienced Primary Care NP or PA to join our outpatient primary care team. The role offers clinical autonomy with access to technology and resources to deliver patient‑centered care.

The position requires MA licensure, national board certification, and a Master’s degree in an NP/PA program. You’ll collaborate with physicians and support teams to improve outcomes and patient experience.

Qualifications

• Master’s degree or higher from an accredited NP/PA program.
• Current Massachusetts licensure as a Nurse Practitioner or Physician Assistant with national board certification.
• Experience practicing in outpatient primary care or family medicine.

Responsabilités

• Provide comprehensive primary care to patients across a broad range of ages.
• Perform patient histories, physical examinations, health assessments, and diagnostic evaluations.
• Diagnose and treat acute illnesses and manage chronic conditions.
• Develop individualized treatment plans and prescribe medications as allowed by MA law.
• Provide preventive care, health counseling, and wellness guidance.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Collaborate with physicians, nurses, medical assistants, and other team members.
• Support care coordination and telehealth when appropriate.
