Job summary

Healthonomic Primary Care in Orland Park and Palos Heights seeks a Nurse Practitioner to join our patient-centered team. You will deliver primary care to an adult panel, focusing on chronic disease management and preventive care alongside our physician group.

Qualifications

• Active Illinois APRN license.
• FNP or AGNP national certification (ANCC or AANP).
• Active DEA and Illinois controlled-substance registration, or eligible before start.

Responsibilities

• Provide primary care to adult patients, including chronic disease management.
• Perform Medicare Annual Wellness Visits and preventive care visits.
• Support chronic care management program with patient care plans and follow-up.
• Order and interpret labs and imaging; manage medications and referrals.
• Document care in Epic and support quality and preventive care initiatives.
