Job Description

Collaborate with interdisciplinary teams to organize, prepare, and deliver evidence-based clinical services across the continuum of care. Uphold ethical principles and professional standards in practice.

  • Perform clinical diagnoses and conduct comprehensive bio-psychosocial assessments to establish care plans.
  • Implement integrated care model treatment guidelines by providing approximately 6-8 sessions of short-term, focused treatment.
Qualifications
  • Master’s degree in social work or related field required.
  • Licenses: LCSW, LPC, LMFT, or LADC required.
  • Prior experience in a medical setting preferred.
  • Bilingual skills preferred.
