Job summary

Hartford Healthcare’s Primary Care Behavioral Health team seeks a licensed behavioral health professional to provide integrated mental health support within a collaborative primary care setting. You will conduct brief assessments, deliver evidence-based interventions, coordinate care with physicians and nurses, and assist patients dealing with depression, anxiety, and chronic disease management.

Qualifications

• Active state license in social work, counseling, psychology or related behavioral health field.
• Certification or formal training in evidence-based therapies (CBT, MI) preferred.
• Prior experience in integrated primary care or collaborative care model preferred.

Responsibilities

• Provide behavioral health assessments for primary care patients
• Deliver brief, evidence-based interventions (e.g., CBT, MI)
• Collaborate closely with primary care providers and care teams
• Develop and update patient-centered treatment and care plans
• Document all encounters in the EMR accurately and timely
• Participate in case conferences and interdisciplinary team meetings
• Support crisis assessment and intervention as needed
• Educate patients and families on mental health and wellness resources
• Coordinate referrals to specialty mental health and community resources
