Avance Care is hiring for an Advanced Practice Provider at a NW Raleigh Primary Care clinic. This full-time role includes rotating 12-hour shifts and weekends, with a focus on comprehensive primary care, acute and chronic disease management, and collaborative care with an integrated team.

You will deliver in-person visits and telehealth as needed, while maintaining a patient-centered approach. Ideal candidates have NC licensure (FNP-C/BC or AGPCNP-BC for NPs; PA-C for PAs) and at least 2 years

Qualifications

• Active North Carolina licensure and certification (FNP-C, FNP-BC, AGPCNP-BC for NPs; PA-C for PAs)
• 2+ years of experience in primary care or family practice
• Commitment to diversity, inclusion, and preventive care

Responsabilités

• Comprehensive primary care: routine exams, preventive screenings, wellness visits.
• Acute and chronic care management: diagnose and treat illnesses and manage chronic conditions.
• Order, perform, interpret tests and prescribe therapies as appropriate.
• In-office procedures within license scope and settings.
