Avance Care is seeking an Advanced Practice Provider for Apex Primary Care. The role offers premium compensation, with a target of a balanced schedule and productive collaboration with on-site labs, behavioral health, and nutrition professionals.

Expect a 40-hour work week, with 36 patient-facing hours and 4 admin hours. You will deliver comprehensive primary care, manage acute and chronic conditions, and coordinate care within an integrated team.

Formación

• 2+ years of experience in a primary care or family practice setting, preferred.
• Active NC licensure and certification (FNP-C, FNP-BC, AGPCNP-BC for NPs; PA-C for PAs) required.

Responsabilidades

• Provide comprehensive primary care including preventive health and wellness visits.
• Diagnose and manage acute illnesses and chronic conditions.
• Order, perform, and interpret diagnostic tests and prescribe medications and therapies.
• Perform minor in-office procedures within scope of practice.
• Collaborate with onsite behavioral health therapists and nutritionists.
• Deliver care via in-person visits and integrated telehealth platforms.
• Educate patients on prevention, lifestyle, and treatment plans.
• Assist with care coordination and electronic documentation in the EHR.

Your Day-to-Day Responsibilities:

Clinical Care & Assessment:

• Comprehensive Primary Care: Perform routine physical exams, preventive health screenings, and annual wellness visits for diverse patient populations.
• Acute & Chronic Care Management: Diagnose and treat acute illnesses while actively managing chronic conditions.
• Diagnostic & Treatment Plans: Order, perform, and interpret diagnostic tests and prescribe safe, effective medications, and/or therapies.
• In-Office Procedures: Execute minor outpatient clinical procedures appropriate to the primary care setting and within the provider's licensed scope of practice.

Integrated & Mindful Medicine:

• Interdisciplinary Collaboration: Actively partner with onsite behavioral health therapists and nutritionists to provide seamless, whole-person care during patient visits.
• Telehealth Delivery: Seamlessly transition between in-person visits and integrated telehealth platforms to support flexible patient access.
• Patient-Centered Education: Counsel and educate patients on preventative lifestyles, proactive chronic disease monitoring, and collaborative treatment plans.

Care Coordination & Administration:

• Care Coordination Partnership: Collaborate closely with Avance Care's centralized care coordination and population health management teams to bridge gaps in patient care.
• Electronic Documentation: Efficiently manage and accurately document patient encounters within the electronic health record (EHR) system during designated administrative blocks.

What Are We Looking For?

• Education & License: Active North Carolina state licensure and certification (FNP-C, FNP-BC, or AGPCNP-BC for Nurse Practitioners; PA-C for Physician Assistants), required.
• Experience: 2 or more years of experience in a primary care, or family practice setting, highly preferred.
