Job summary

OSF HealthCare is seeking a Nurse Practitioner or Physician Assistant to join an established Primary Care practice in Illinois. You will perform patient assessments, assign diagnoses, and establish care plans in line with care paths and current research.

The role includes patient education and counseling, with clinic hours Monday–Friday from 8am to 5pm. Applicants should hold an advanced nursing or PA degree, an Illinois NP/PA license, and ANCC/NCCPA certification, with a preference for 2+ years

Qualifications

• Master’s degree or higher in nursing or physician assistant studies from an accredited program.
• Current Illinois license as a nurse practitioner or physician assistant
• Certification by the ANCC or NCCPA (NCCPA)
• Minimum of two years of APP experience in primary care or related field is preferred
• BLS and ACLS certifications
• Proficiency in electronic medical record (EMR) systems

Responsibilities

• Join an established Primary Care practice.
• Perform patient assessment and assign diagnosis based on recorded assessment data and current scientific guidance.
• Establish plan of care and implement interventions per care paths and protocols.
• Provide education and counseling to patients, families and groups.
• Clinic hours are Monday – Friday 8am-5pm
• Excellent work/life balance with no call or weekends
