Job summary

Mount Carmel Health System in Johnstown, Ohio is seeking a full-time, experienced Advanced Practice Provider to join a busy primary care practice. The APP delivers patient-centered care across the lifespan with emphasis on health promotion, disease prevention and improving community access to care.

Requirements include a Master’s degree, Ohio APRN license, BLS and DEA; 1–2 years outpatient experience preferred.

Qualifications

• Master's degree in science, nursing or specialty field, and graduate of Advanced Practitioner Program.
• Current Ohio RN license and APRN certification in designated specialty.
• BLS provider certification and DEA license.

Responsibilities

• Manages the care and treatment of patients within standards and protocols.
• Develops a comprehensive plan of care with the patient/family.
• Practices under a Standard Care Arrangement with a collaborating physician.
• Initiates and leads performance improvement activities to improve clinical indicators.
