Job summary

National Church Residences is seeking a Nurse Practitioner to deliver comprehensive primary care to residents across the Dayton metro area, from wellness visits to chronic disease management.

You will round in the field four weekdays with one administrative day at home, focusing on care in skilled nursing, long term care, assisted living, and independent living facilities.

Qualifications

• MSN required with NP board certification.
• Active Ohio RN and APRN-CNP licenses in good standing.
• 2+ years of RN experience preferred.
• Valid driver’s license and reliable transportation.
• Experience in long term care, geriatrics, or primary care.

Responsibilities

• Provide comprehensive primary care including exams, histories, diagnosis and treatment of acute and chronic conditions.
• Order and interpret labs, prescribe medications, and order therapies such as PT and dietary changes.
• Build care plans accounting for social/economic circumstances and connect patients with community resources.
• Serve patients in skilled nursing facilities, long term care, and assisted living and determine referrals or hospital admissions when needed.
• Educate patients about prevention and medications and follow up as needed.
• Contribute to clinical guidelines and health education materials; represent NCR at health fairs and community programs.
