Vanderbilt University Medical Center in Nashville is seeking an Advanced Practice Provider to collaborate with a multidisciplinary team in the outpatient Internal Medicine setting. The APP will manage a defined patient population with emphasis on general, episodic, and preventive care, adhering to clinical pathways and maintaining high-quality patient care.

Qualifications

• Master’s degree required in nursing or related field.
• State license to practice as a Nurse Practitioner or Physician Assistant in Tennessee.
• Clinical expertise in outpatient primary, acute, or preventive care.

Responsibilities

• Obtains and documents health history; performs physical examinations and develops care plans.
• Provides healthcare services for primary, acute and complex care and manages patient care outcomes.
• Orders, performs, and interprets diagnostic tests as indicated.
• Formulates differential diagnoses and treats acute and chronic conditions.
• Prescribes pharmacologic and non-pharmacologic therapies to achieve outcomes.
• Uses evidence-based protocols in planning and implementing care.
• Initiates referrals and consultations as needed.
• Provides specialty-specific consultation within scope of practice.
• Facilitates patient transitions between care settings.
• Acknowledges that duties may evolve and additional tasks may be assigned.
