Job summary

Abington Memorial Hospital in Warrington, PA, is seeking a qualified Advanced Practice Clinician (APC) for a full-time Primary Care position. The APC will perform histories and physicals, document progress, order and interpret diagnostic tests, and collaborate with the care team across the inpatient and outpatient continuum.

The ideal candidate holds a Master’s Degree with national board certification as a Primary/Family NP or PA, plus BLS.

Qualifications

• Master’s Degree required and national board certification as NP or PA.
• BLS certification required.
• Strong collaboration with interdisciplinary teams.

Responsibilities

• Performs complete history & physicals and documents in the medical record.
• Orders and collects data using appropriate assessment techniques and diagnostics.
• Collaborates with interdisciplinary health care team in making diagnoses of acute and chronic conditions.
• Manages diagnostic tests through ordering, interpretation, and supervision.
• Formulates Plan of Care and implements evidence-based interventions.
• Maintains documentation of progress, discharge summaries, and plan of care.
