The Arc Jefferson - St. Lawrence is seeking a dedicated professional to support prevocational services for individuals with disabilities. You will develop and implement prevocational plans and maintain required program documentation to ensure quality outcomes.

Responsibilities include coordinating with Therap documentation, completing ISP-related plans, attending meetings, and collaborating with families and care teams to promote independence and successful employment outcomes.

Formación

• Bachelor’s Degree in Human Service Field.
• One-year experience with Developmentally Disabled population or vocational setting.
• Valid NYS Driver’s License with acceptable driving history per the agency’s insurance requirements.

Responsabilidades

• Responsible for each assigned Program Participant’s prevocational plan of service and required program documentation.
• Assist People We Support to develop work skills and behaviors to reach their maximum prevocational potential.
• Provide daily interventions with the People We Support and work in cooperation with all program staff to promote Independence, Integration, Individualization and Productivity.
• Complete documentation for annual reviews, semi-annual reviews, and interim meetings.
• Verify daily attendance records and ensure documentation is scanned and uploaded to Therap.
• Complete ISP Plans and Personal Focus Worksheets as necessary in Therap.
• Attend and satisfactorily complete all required in-services.
