The Prevention Family Specialist aims to preserve the family unit by providing supportive casework counseling in the home, empowering parents with tools and resources to ensure children's safety, well-being, and permanency, and advocating for quality services.

Job Responsibilities
- Conduct assessments of family strengths and needs; identify problem areas and utilize various treatment modalities and resources to promote growth and change.
- Develop, implement, and evaluate comprehensive treatment and discharge plans. Explore alternative placement or permanency options for youth not returning to their families. Complete a discharge summary at least eight weeks prior to discharge.
- Deliver effective case management services, ensuring counseling sessions are scheduled and children receive necessary medical and dental examinations. Track services, complete documentation timely, and ensure compliance with confidentiality, parental consent, and information release standards.
- Engage with families through casework contacts at least twice a month in homes and community settings, focusing on treatment goals. Build effective, caring relationships with youth and families. Ensure children's safety and develop relationships with community schools and resource agencies.
- Maintain relationships with community schools and resource agencies to support the educational and overall well-being of children.
Job Requirements
- Bachelor’s degree in human services or a related field.
- Minimum of 1 year experience working with at-risk youth and families, including case management.
- Flexible schedule including day, evening, weekend, holiday, and call hours.
- On-call responsibilities in collaboration with a Program Coordinator, possibly including evening and weekend fieldwork.
- Valid driver’s license, reliable vehicle, and ability to travel for visits and appointments.
- Ability to work with diverse populations culturally and economically.
- Strong crisis intervention, communication, and listening skills. Bilingual skills are a plus.
