Potential Title -Prevention Care Coordinator

Reports To: Director of Health Outreach Services

Job Type: Full-Time, regular, hourly

Firm Rate: $22.50

Summary

The Prevention Care Coordinator serves as the first point of contact for many clients accessing prevention services. This position combines patient registration, front office operations, electronic medical record (EMR) management, data entry, and administrative coordination with direct client services, including rapid testing and prevention navigation. The ideal candidate is highly organized, detail-oriented, technologically proficient, and passionate about providing exceptional client-centered care in a welcoming, affirming environment. Position is in-person at work locations interacting with the public.

Essential Duties and Responsibilities
  • Client Services & Front Office Operations -Serve as the first point of contact for clients accessing AVOL Prevention Services by creating a welcoming, affirming, and professional experience while coordinating patient flow and administrative processes.
  • Electronic Medical Record (EMR) Management - Maintain accurate and complete patient records within AthenaOne through patient registration, appointment management, insurance verification, documentation support, and ongoing quality assurance.
  • Prevention Services Support - Provide HIV, STI, hepatitis C, syphilis, and pregnancy testing, prevention education, PrEP and PEP navigation, and linkage to care following AVOL policies and training requirements.
  • Data Management & Quality Assurance - Support the integrity of prevention program data through accurate documentation, REDCap data entry, duplicate record review, grant reporting support, and continuous quality improvement activities.
  • Program & Administrative Support - Support the daily operations of the Prevention Department through inventory management, supply coordination, outreach event preparation, and collaboration with clinical and administrative staff.
General Duties
  • Proven comfort level working with marginalized populations and the diversity of people impacted by HIV and other chronic health conditions.
  • Demonstrate professionalism, compassion, and confidentiality in all client interactions while maintaining compliance with HIPAA and organizational policies.
  • Accurately document patient encounters, testing services, referrals, and administrative activities in accordance with organizational policies and grant requirements.
  • Collaborate effectively with Prevention Specialists, medical providers, patient navigators, and community partners to ensure seamless client care and continuity of services.
  • Represent AVOL professionally during community outreach events, health fairs, and public education activities as assigned.
  • Participate in departmental meetings, quality improvement initiatives, and required trainings.
  • Perform other duties as assigned to support the goals and mission of the Prevention Department including some work hours that work best for our client populations including evening, and weekends scheduled in advance.

Minimum Qualifications
  • High school diploma or equivalent required.
  • Demonstrated ability to provide compassionate, culturally responsive, and trauma-informed customer service to individuals from diverse backgrounds.
  • Strong organizational skills and attention to detail with the ability to manage multiple priorities. Proficiency with computers and common office software, with the ability to quickly learn electronic medical record (EMR) systems and other healthcare technology.
  • Ability to maintain strict confidentiality and comply with HIPAA and organizational policies.
  • Ability to work independently while contributing effectively as a member of a multidisciplinary team.
Preferred Qualifications
  • Associate's or Bachelor's degree in Public Health, Healthcare Administration, Human Services, Social Work, Psychology, Nursing, Medical Assisting, or a related field.
  • Experience using AthenaOne, REDCap, or other electronic medical record and clinical data management systems.
  • Experience in public health, HIV/STI prevention, medical office administration, patient registration, healthcare operations, or community health - lab services and phlebotomy are a plus but not required.
  • Experience working with individuals living with or disproportionately impacted by HIV, hepatitis C, sexually transmitted infections, substance use, or other underserved populations.
  • Bilingual proficiency in English and Spanish or experience serving multilingual communities is strongly preferred - includes a 2.5% pay differential for bilingual staff.
  • Established relationships or experience working with populations disproportionately impacted by HIV or other communicable diseases is a plus.
  • Certifications such as CNA, Medical Assistant, pharmacy experience, peer support specialist are a plus.
