Job summary

Maryland Care Management, Inc. (MCMI) seeks a dedicated Prevention and Wellness Educator to coordinate and deliver prevention education activities to Maryland Medicaid recipients. The role supports outreach to promote preventive services and facilitates access to care across the state.

You will develop health education opportunities, collaborate with interdepartmental partners, and track program outcomes while supporting ongoing quality initiatives and newsletters.

Qualifications

  • Background in managed care, case management, or population health.
  • Experience in health education or outreach in underserved communities.
  • Public speaking, creative writing, and graphic arts experience is preferred.

Responsibilities

  • Conduct outreach to Medicaid recipients and network providers to promote preventive services.
  • Build an ongoing calendar of events for health education with internal/external partners.
  • Target calls to recipients with no recent visits to a practitioner.
  • Identify barriers to care and coordinate transportation or appointment scheduling.
  • Educate recipients on preventive healthcare via virtual, telephonic, and onsite presentations.
  • Support quality initiatives and regulatory/accreditation requirements (NCQA, EQRO, HEDIS).
  • Maintain and update a resource library for programs and events; develop materials.
  • Track outreach activities and monitor outcomes via surveys.
  • Assist with producing quarterly newsletters and write/review articles.

Summary/Position Objectives:

Under the direction of the Manager, this position is responsible for coordinating and delivering prevention and wellness education activities to Maryland Medicaid recipients. The Prevention and Wellness Program is specifically designed to promote the importance and benefits of accessing timely preventive and well care through targeted member/provider education and outreach initiatives.

What You'll Do:
  • Conducts outreach to Medicaid recipients and network providers to remind and promote utilization of preventive services.
  • Finds and/or creates preventive and educational opportunities and events through research and collaboration with interdepartmental, local, state, and federal entities to build an ongoing calendar of events serving members and providers. This includes determining appropriate activities to attend to provide health education based on P&W Work Plan goals.
  • Targets phone calls to Medicaid recipients without recent visits with a healthcare practitioner.
  • Identifies barriers to accessing healthcare services and takes appropriate action to address such obstacles (i.e., appointment scheduling, coordination of transportation services) by making referrals to Care Management.
  • Educates Medicaid recipients on the importance of preventive healthcare through virtual, telephonic, and onsite health education presentations, informational tables, health fairs, community baby showers, and other venues as needed.
  • Supports quality initiatives and regulatory, contractual, and/or accreditation requirements including but not limited to NCQA, EQRO, and HEDIS.
  • Maintains and updates a resource library of educational programs/resources and develops materials/tools for specific sponsored programs, events, and Community Advisory Board meetings.
  • Tracks and reports on outreach and educational activities and monitors outcomes of educational presentations through surveys every month and as needed by supervisor.
  • Assists with coordinating the production of the quarterly member and provider newsletters and assists with writing and reviewing articles as applicable.
Secondary Functions:
  • Must work in a manner that is not disruptive to peers, supervisors, and/or subordinates.
  • Must be available and willing to work and travel throughout MD, which includes some evenings and weekends.
Requirements
Knowledge, Skills, and Abilities:
  • Background in managed care, case management, or population health.
  • Effective communication skills, public speaking skills, and influencing skills that can be applied to a broad internal/external audience.
  • Ability to successfully utilize Microsoft Office suite.
  • Strong problem-solving, organizational, and time management skills.
  • Must have own reliable transportation with appropriate insurance and willingness to do statewide travel.
Education and Work Experience:
  • Associate or bachelor’s degree in a health-related field preferred.
  • Must have a valid driver's license.
  • Demonstrated experience in a managed care environment with a minimum of one year of recent experience with health education, outreach, or working with local health departments, agencies, and/or organizations in underserved communities.
  • Experience with public speaking, creative writing, and graphic arts is preferred.
