Job description

About Us: The Minnesota River Area Agency on Aging (MNRAAA) provides advocacy, information, resources and assistance so that older adults can maintain the lifestyle of their choice. The Senior LinkAge Line is a free statewide service of the Minnesota Board on Aging, in partnership with Minnesota's Area Agencies on Aging. Senior LinkAge Line helps older Minnesotans and their families by connecting to local services, finding answers, and getting the help they need.

Job Summary: MNRAAA is seeking a Senior LinkAge Line preadmission screening specialist. Federal and state laws require preadmission screening for all people prior to entering a Medical Assistance-certified nursing facility, certified boarding care facility or hospital swing bed. The Senior LinkAge Line receives all preadmission screenings and is responsible for processing or triaging it. Job responsibilities include answering preadmission screening phone calls and chats, providing virtual outreach to providers and lead agencies, documenting, and faxing or emailing paperwork.

Location: Must live within MNRAAA's SW MN (27-county service area)

Position Type: Remote: Full-Time (40hrs)/Non-exempt

Salary And Benefits

  • Salary range: $21.28-$25.29 per hour depending on years of relevant experience.

Key Responsibilities

  • Answer inbound phone calls and chats from providers or lead agencies with questions or updates about preadmission screening.
  • Document phone calls or chats accurately, concisely, and timely after each interaction.
  • Review the state's MMIS and MAXIS systems and either process or triage preadmission screening or level of care redetermination to the appropriate lead agency.

Qualifications

  • Requires a bachelor's degree from an accredited college or university in a human services-related field, such as social work, nursing, public administration, or gerontology and one year of professional or internship experience working with older adults
  • OR Two-year certificate or degree in a human services-related field or a community health certificate with three years of professional experience working with older adults
  • OR Five years of relevant professional experience in care coordination and case management at a nursing facility, assisted living, hospital, home care or lead agency.
