Qualifications

• Nursing diploma or associate degree in nursing.
• RN license/certification required.
• Preferred: Bachelor's degree in Nursing.
• Preferred: less than 2 years of experience.

Responsibilities

• Promotes patient and family centered care in a healing environment.
• Educates patients and families on illness management, home care needs and medications.
• Participates in activities that promote patient safety, quality and regulatory compliance.
• Develops, implements, and documents plans of care with defined goals in collaboration with the interprofessional team.
• Proactively plans and ensures communication of the plan of care across the continuum of care.
• Uses nursing skills to assess physical, psychosocial, and emotional needs.
