Responsibilities

1. Process all requests for services according to Medicare and NYS Medicaid Guidelines, as well as departmental criteria and guidelines.
2. Collaborate with Care Managers, Assessment Nurses, Primary Care Physicians, and Specialists to ensure timely provision of services.
3. Coordinate timely completion of UAS NY assessments reflecting the current condition of members.
4. Ensure that the Individualized Person-Centered Care Plan is updated and accurately reflects the member's current status.

Qualifications

• Graduate of an accredited nursing program; Bachelor’s degree preferred.
• Knowledge of Utilization Management processes preferred.
• Current NY RN license and registration required.
• CCM certification preferred.
• Minimum of three to five years of nursing experience in long-term care, managed care, home care, geriatrics, or hospice.
• Experience working in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations.
• Proficiency in Windows, Word, and Excel.
• Strong assessment skills and knowledge of current standards of medical practice and healthcare delivery systems.
• Knowledge of Interqual criteria preferred.
