An established industry player is seeking a dedicated FCT Practitioner to provide in-home treatment aimed at supporting families in recovery. This role involves collaborating with clients and their families to develop personalized treatment plans, conducting regular sessions, and maintaining detailed records. The ideal candidate will possess a Master’s degree in social work, have strong organizational and communication skills, and be committed to fostering positive relationships with diverse populations. You will complete the Wheels of Change and field training to become a certified FCT and FCT-R Practitioner. The FCT Practitioner will provide in-home treatment with the intent to keep families together, based on the program’s model. You will support families with members who are on their recovery journey by participating on a team with a Peer Recovery Specialist to support family members in working toward recovery and maintaining their family in the home.

This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop and/or cell phone, and communicating with colleagues.

Statewide travel required.
