An established industry player is seeking a dedicated Practitioner to join their team. This role focuses on providing in-home treatment to families, ensuring they receive the support they need to stay together. With a strong emphasis on collaboration, you'll work closely with clients, their families, and other professionals to create tailored treatment plans. The ideal candidate will have a Bachelor's degree in social work and at least three years of relevant experience. What You’ll Be Doing

You will complete the Wheels of Change and field training to become a certified FCT Practitioner. The FCT Practitioner will provide in-home treatment with the intent to keep families together, based on the program’s model.

Your Main Priorities
  • Work with clients and their family members and caregivers to identify opportunities and goals, develop a treatment plan, and assist in achieving desired outcomes.
  • Build and maintain positive relationships with clients, family members and caregivers, and identify referring or collateral professionals and agencies.
  • Collaborate with collateral contacts as needed.
  • Provide two 2-hour treatment sessions per week with the appropriate individuals.
  • Be flexible in providing services according to the client’s needs.
  • Maintain client records accurately and complete documentation requirements timely.
Who You Are

Requirements, Skills & Abilities:

  • Bachelor’s degree in social work or related field required.
  • Minimum 3 years’ experience in a similar role required.
  • Experience with trauma-informed treatment is a plus.
  • Bilingual in Spanish preferred.
  • Strong organizational, critical thinking, and time-management skills.
  • Basic computer skills required; knowledge of Microsoft Office 365.
  • Ability to communicate and effectively interact with a diverse population.
  • Collaboration and teamwork skills.
  • Must have a valid driver’s license, proof of auto insurance, and car registration.
  • Must authorize Child & Family to complete a motor vehicle records check.
  • The candidate must meet all background checks and medical clearances required for the position.
Physical and Travel Requirements

This position requires sitting, standing, walking, driving, carrying, and lifting up to 25 lbs. It also involves operating desktop, laptop, and/or cell phone, and communicating with colleagues.

Site location: Providence. Statewide travel required.
