Job description

Practice Transformation Specialist, Nebraska

Join Aledade as a Practice Transformation Specialist (PTS) and make a tangible impact on patient care. As a PTS, you will be the primary interface with healthcare providers, gaining insights into practice needs and satisfaction. The role requires a passion for population health and a commitment to providing better, cost-effective care to patients. Transforming one practice at a time, you will contribute to community health improvement.

Primary Duties:
  1. Direct Practice Support: Implement products and execute ACO initiatives under guidance from Market Leadership. Support wellness visits, care transitions, and active management to meet Practice Transformation Plans.
  2. ACO Training & Development: Assist in training office staff and providers on processes and technologies, and assess ongoing training needs. Build lasting relationships with physicians, providers, and healthcare executives to improve performance metrics.
  3. Customer Success: Address practice concerns, communicate insights to management, and identify opportunities for improvement.
Minimum Qualifications:
  • Resident of Michigan willing to travel up to 25% within your territory.
  • Graduated from an accredited college or university, or have 4-6 years relevant clinical practice management experience.
  • At least 3 years of experience with Electronic Health Records (EHR) systems.
Preferred Skills and Abilities:
  • Excellent communication, organizational, and project management skills.
  • Familiarity with process mapping and workflow analysis tools.
  • Strong computer skills and willingness to learn new software.
Physical Requirements:
  • Sitting for long periods, extensive computer use, occasional walking and lifting, and ability to travel 10-25% as needed.
