Job description

Join Aledade as a Practice Transformation Specialist (PTS) and make a direct impact on patient care. Practice Transformation Specialists are the primary interface with providers and gain valuable insights into partner practices' needs and satisfaction. A successful PTS is passionate about population health and dedicated to providing better care while reducing costs. At Aledade, a PTS can influence community health by transforming practices one at a time.

Primary Duties:
  1. Direct Practice Support:
    • Implement products and execute ACO initiatives under the guidance of Market Leadership.
    • Support wellness visits, care transitions, active management, and ensure practices meet goals outlined in Practice Transformation Plans.
  2. ACO Training & Development:
    • Assist in training office staff and providers on processes and technologies in coordination with Aledade's Value Based Care Curriculum.
    • Continuously assess practice and staff training needs.
    • Build and maintain relationships with physicians, providers, and healthcare system leaders to support financial and quality performance metrics.
  3. Customer Success:
    • Listen to practice concerns and communicate insights to managers to identify opportunities for improvement.
Minimum Qualifications:
  • Resident of Michigan willing to travel up to 25% within the territory.
  • Graduate of an accredited college or university, or equivalent relevant experience in medical practice management, especially in administrative roles within clinical settings.
  • At least 3 years of experience with Electronic Health Records (EHR) related to clinical or practice management.
Preferred Skills and Abilities:
  • Excellent communication, organizational, project management, and team-building skills.
  • Familiarity with process mapping and workflow analysis tools.
  • Strong computer skills and willingness to learn new software applications.
Physical Requirements:
  • Sitting for extended periods.
  • Extensive computer and keyboard use.
  • Occasional walking and lifting.
  • Ability to travel 10-25%, depending on the market.
