Practice Transformation Specialist, Louisiana

Join Aledade as a Practice Transformation Specialist (PTS) and make a direct impact on patient care. PTSs are the primary interface with providers, gaining insights into practice needs and satisfaction. A successful PTS is passionate about population health and committed to improving patient care while reducing costs. Transform one practice at a time to impact community health.

Primary Duties:

1. Direct Practice Support: Implement products and ACO initiatives, support wellness visits, care transitions, and active management, ensuring practices meet agreed-upon transformation goals.
2. ACO Training & Development: Assist in training staff and providers on processes and technologies, assess training needs, and foster relationships with healthcare professionals to improve performance metrics.
3. Customer Success: Address practice concerns, communicate opportunities for improvement, and support practice success.

Minimum Qualifications:

• Resident of Louisiana willing to travel up to 25% within the territory.
• College graduate or equivalent experience in medical practice management, with significant administrative experience in clinics accepted.
• At least 3 years of experience with Electronic Health Records (EHR) in clinical or practice management.

Preferred Skills:

• Excellent communication, organizational, and team-building skills.
• Experience with process mapping and workflow analysis tools.
• Strong computer skills and willingness to learn new software.

Physical Requirements:

• Prolonged sitting and extensive computer use.
• Occasional walking, lifting, and travel (10-25%).
