Job Description

Accounts for the development, implementation and delivery of clinical quality initiatives and related activities for the Lurie Children’s Health Partners Clinical Integration Network (CIN). Collaborates with a broad group of stakeholders, including but not limited to the CIN leadership, vendors and other consultants, Lurie Children’s leadership, CCPA physician members/practices and FPP physician members/practices. Will also have responsibility for developing and enhancing provider relationships, in addition to the recruitment of additional providers and the ongoing liaison role with community-based physicians and their practice staff.

Job Duties:

  • Participates in the implementation of a strategically driven and structured approach to integration and improvement of care across the network.
  • Supports the network in transitioning to value-based payment structures that promote achieving our mission. Develops meaningful summary reports that support the current clinical metrics for the CIN; possesses working knowledge of the payor contracts and any specific incentive models.
  • Leads physician and practice staff training regarding updates in cost, utilization, quality and patient experience performance. Serves as ‘the face’ of the CIN in the physician offices and the primary point of contact for questions/issues.
  • In conjunction with CIN vendors, supports the implementation and ongoing data acquisition in support of the clinical quality measures, trains providers on vendor-provided tools, and works to troubleshoot issues identified by practices in the tools provided by the CIN.
  • Project manager for CIN service offerings including but not limited to the pediatrician/specialist peer to peer communication tool, patient experience survey, and patient outreach program.
  • Educates CIN providers in value-based principles such as cost, quality, and patient experience metrics, specialty retention reports, market analysis, patient-provider assignment, and contracting.
  • Assists in the planning and coordination of CIN network-wide events such as the Annual Forum, Regional Meeting, or webinars; supports marketing of services and events to network providers.
  • Attends CIN meetings as requested by leadership.
  • Performs job functions adhering to service principles with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.
  • Other job functions as assigned.

Minimum Qualifications:

  • Bachelor’s degree required. Master’s degree (MPH, MHA, MSN and MBA) preferred.
  • Understanding of quality improvement and integration of care across the continuum preferred.
  • Minimum of 3 years of related professional experience required.
  • Demonstrated knowledge and experience in utilizing quality measurement systems and in creating practical and effective practice workflow tools.
  • Proven relationship skills with physicians and medical practice personnel required.
  • Strong verbal and written communication skills required.
  • Strong computer skills; Microsoft and other applications; EMR knowledge strongly preferred.
  • Local travel required; ability to attend evening meetings required.

Pay Range

$65,520.00-$107,120.00 Salary
