The @HOME program delivers personalized primary care to Medicare patients in their homes through a team-based approach, including providers, medical assistants, behavioral health specialists, and mobile operations coordinators, managing about 75 patients each month.
The Practice Manager leads market operations to ensure excellent patient care and a positive work environment. Alongside the Medical Director, the manager oversees clinical and non-clinical duties, supports operational initiatives, and drives team performance towards patient satisfaction and clinical outcomes.
Required: 2+ years supervisory experience, proficient PC skills, US work authorization, embodying “Oaky” values.
Preferred: Bachelor’s degree, 5+ years supervisory experience in clinical settings, experience with Google Suite, and driving team metrics.
Position details include 40 hours/week, full-time employment, with a pay range of $46,988 - $102,000, plus benefits such as health insurance, 401K, and career growth opportunities.
