Job summary

Impact Life, Inc. in Dover, Delaware is seeking a Practice Coordinator to lead the Kent County Social Prescribing & Integrated Outpatient Services program.

The role unites medical care, behavioral health, peer recovery, Social Prescribing, and community services to create coordinated, person-centered pathways to care. The ideal candidate will have a Bachelor's degree and at least three years in behavioral health or care coordination, with strong leadership, communication, and data reporting

Qualifications

  • Bachelor's degree in social work, behavioral health, public health, human services, healthcare administration, or a related field.
  • Minimum of three years of relevant experience in behavioral health, substance use disorder services, recovery services, community health, care coordination, Social Prescribing, or a related human-services setting.
  • Experience supervising, coaching, or providing day-to-day leadership to staff.
  • Strong understanding of substance use disorder, recovery-oriented systems of care, social determinants of health, and person-centered services.
  • Strong organizational, communication, documentation, problem-solving, and relationship-building skills.
  • Ability to travel locally throughout Kent County and to community or partner locations as required.

Responsibilities

  • Oversee day-to-day implementation and operations of the Social Prescribing & Integrated Outpatient Services program.
  • Develop and coordinate workflows that support low-barrier access, participant engagement, treatment retention, and continuity of care.
  • Coordinate services across medical, behavioral health, peer recovery, Social Prescribing, and community-based systems.
  • Support individualized Social Prescribing pathways connecting participants with healthcare, recovery supports, wellness activities, community resources, and other non-clinical supports.
  • Facilitate warm handoffs and effective connections to treatment, healthcare, housing, transportation, recovery support, and community resources.
  • Collaborate with the Nurse Practitioner, Peer Recovery Specialist/Link Worker, behavioral health providers, and community partners.
  • Provide supervision, guidance, coaching, and day-to-day leadership to assigned staff.
  • Participate in recruitment, onboarding, training, performance management, and professional development.
  • Build and maintain partnerships with healthcare, behavioral health, SUD treatment, recovery, and community organizations.
  • Strengthen referral pathways and support outreach, education, participant recruitment, and community awareness.
  • Monitor participant engagement, retention, referrals, linkages, and required program performance measures.
  • Coordinate data collection and reporting required by leadership, funders, and applicable contracts.
  • Monitor program documentation for timeliness, quality, completeness, and compliance.
  • Lead and participate in quality improvement initiatives.
  • Ensure program activities align with applicable grant and contract requirements.

About the Role

Impact Life is seeking an experienced and collaborative Practice Coordinator to provide day-to-day leadership and coordination for our Kent County Social Prescribing & Integrated Outpatient Services program.

This position oversees an integrated, community-based model designed to improve treatment engagement and access to comprehensive recovery supports for adults with opioid use disorder, stimulant use disorder, co-occurring behavioral health conditions, and complex social needs.

The Practice Coordinator brings together medical care, behavioral health, peer recovery, Social Prescribing, and community-based services to create coordinated, person-centered pathways to care. The role also provides staff leadership and is responsible for program implementation, quality improvement, partnership development, reporting, and contract compliance.

Key Responsibilities

Required Qualifications

  • Demonstrated experience coordinating programs, services, or multidisciplinary care.
  • Experience supervising, coaching, or providing day-to-day leadership to staff.
  • Strong understanding of substance use disorder, recovery-oriented systems of care, social determinants of health, and person-centered services.
  • Strong organizational, communication, documentation, problem-solving, and relationship-building skills.
  • Ability to manage multiple priorities, monitor deliverables, and meet reporting and contract deadlines.
  • Ability to collaborate with medical, behavioral health, peer recovery, and community partners.
  • Ability to travel locally throughout Kent County and to community or partner locations as required.
Preferred Qualifications
  • Master's degree in social work, behavioral health, public health, healthcare administration, or a related field.
  • Experience with Social Prescribing, integrated care, outpatient behavioral health, SUD treatment, or peer recovery services.
  • Experience with grant- or contract-funded programs, including performance monitoring and reporting.
  • Experience with quality improvement and data-informed program management.
  • Knowledge of Kent County and Delaware healthcare, behavioral health, recovery, and community resources.
  • Experience using an Electronic Health Record (EHR).
