Job summary

Capital Area Human Services District seeks a compassionate Licensed Practical Nurse to join the Capital Area Recovery Program (CARP) in Baton Rouge. Qualifications

• Licensed Practical Nurse license in Louisiana or eligibility for NLC/eNLC.
• Current license, temporary permit, or multi-state license recognized for practice.
• Ability to perform 12-hour night shifts and weekends as scheduled.

Responsibilities

• Triage clients for ongoing medical needs and coordinate referrals and follow-up care.
• Provide emergency nursing care and facilitate access to treatment when needed.
• Observe, assess and notify physician of changes in mental or physical condition.
• Process admissions, transfers, and discharges with physician orders documentation.
• Complete nursing assessments and develop individualized plans of care.
• Maintain accurate nursing documentation: daily/monthly reports, shift and weekly summaries.
• Participate in multidisciplinary treatment team meetings and care plan reviews.
• Review and document physician orders in the electronic health record (EHR).

Possession of a current Louisiana Practical Nurse license, a temporary permit, or a multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state.

Job Duties:

• Triage clients for ongoing medical needs and coordinate appropriate medical referrals and follow-up care.

• Provide emergency nursing care and facilitate access to appropriate medical treatment when needed.

• Observe and assess client behavior and identify changes in mental or physical condition, including signs of psychiatric or medical deterioration; promptly notify the physician

• Process client admissions, transfers, and discharges, including obtaining and documenting physician orders.

• Complete integrated nursing assessments upon admission and assist with developing individualized nursing plans of care and treatment.

• Maintain accurate and timely nursing documentation, including daily and monthly reports, shift reports, weekly client summaries, progress notes, and other required medical records.

• Participate in multidisciplinary treatment team meetings and contribute to the development and review of client care plans.

• Perform CLIA testing for new admissions and as clinically indicated; promptly report positive results to appropriate personnel and nursing leadership.

• Review, process, and document physician medical and psychiatric orders accurately and timely in the electronic health record (EHR)

Compensation:

This position has a Special Entrance Rate (SER) at each level. However, the salary offered will be compensated based on qualifications and experience.

• LPN 1 -$1,558.40 bi-weekly / $19.48 per hour
• LPN 2 -$1,667.20 bi-weekly / $20.84 per hour
• LPN 3 -$1,784.00 bi-weekly / $22.30 per hour
