Job summary

Easterseals VT is seeking a Post Permanency Services Coordinator to support Vermont families formed through adoption and guardianship. You will implement plans of care, conduct home visits with caregivers, and provide in‑home education on parenting, trauma, and advocacy.

You will collaborate with schools, therapists, and child welfare to sustain strong family outcomes. Ideal candidates have a bachelor’s degree in human services, excellent writing, and reliable transportation.

Qualifications

  • Bachelor’s Degree in human services or related field.
  • Two years of experience preferred.
  • Excellent writing skills.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Write and implement plans of care approved by the consulting agency clinician.
  • Visit client homes to meet with caregivers and ensure plan compliance.
  • Provide in‑home services including parenting education, trauma education, and advocacy.
  • Assist families to access resources and coordinate with school and service teams.
  • Attend PPSP trainings and VT Consortium trainings; maintain catchment‑area support groups.

Job Description:Are you ready to make a meaningful impact? Easterseals VT is seeking a Post Permanency Services Coordinator (Post Adoption and Guardianship Services Coordinator) to empower Vermont families formed through adoption and guardianship by providing compassionate and tailored support. In collaboration with the State Post Permanency Manager, this role delivers family-centered services that focus on normative adoption experiences and early childhood trauma.Individuals with a passion for supporting adoptive and guardianship families, as well as those with lived experience, are strongly encouraged to apply!Responsibilities include:Write and implement plans of care to be approved by the consulting agency clinician. Write contact notes, formal monthly reports and discharge summaries.Visit client homes to meet with caregivers to implement the requirements of the individual’s plan of care and ensure compliance of the case plan.Provide services to family in the home that encompass adjusted parenting education, developmental trauma education, normative adoption development, advocacy, school issues, kinship support, and consultation regarding birth parent contact.Assist the family in finding resources for preventative and supportive services and partner with parents by participating in treatment teams, school teams, coordinated services plan meetings, and other appropriate meetings.Attend PPSP quarterly trainings, a minimum of two additional trainings per year that relate to early trauma or adoption, all VT Consortium trainings and conferences, and all Fostering to Forever classes within their catchment area.Make diligent efforts to establish and maintain support groups for families formed through adoption and guardianship in assigned catchment area. Provide transportation for clients when required. Testify concerning treatment services in court when subpoenaed.Provide in-home training and support in the areas of normative adoption adjustment, family relationships, emotional and behavioral issues, birth family contact, educational advocacy, resource development, therapeutic referrals, trauma recovery and other topics in order to ensure that adoptive and Guardianship parents can permanently care for youth in an effective and positive manner.Provide preventative crisis support in the form of crisis plan development by way of treatment team participation, connecting the family to their designated crisis services and the larger system of care, and/or prompting Coordinated Service Plan meetings.Advocate for youths’ educational services by participating in individual education plan meetings, school behavior planning, and other school-related preparation.Maintain frequent communications with social workers, therapists, school staff and other community partners to promote quality, seamless services.Qualifications:Bachelor’s Degree in human services or related field requiredTwo (2) years of experience preferredExcellent writing skills necessaryValid driver’s license and reliable transportationHours: This is a full-time, 40 hour/week position.Compensation: Starting at $23/hour, with additional compensation based on relevant experience and education.What’s in it for you?
