Job summary

SnapCare in Massachusetts is seeking a dedicated Registered Nurse to deliver comprehensive care in a post-acute setting. You will assess patients, develop and implement care plans, and work with an interdisciplinary team to promote recovery and safety.

Qualifications

• Active RN license is required.
• 1–2 years of nursing experience, preferably in post-acute, rehabilitation, or long-term care.
• CPR/BLS certification is required.

Responsibilities

• Conduct detailed assessments of patient health conditions and monitor ongoing health status.
• Implement individualized care plans in coordination with healthcare providers.
• Administer medications and treatments as prescribed, monitoring for side effects or adverse reactions.
• Provide patient and family education on disease processes, treatment protocols, and self-care.
• Ensure accurate and timely documentation of patient information, including vital signs, progress notes, and medical history.
• Collaborate with physicians, therapists, social workers, and other team members to achieve the best patient outcomes.
• Participate in discharge planning, ensuring a smooth transition to home care or other appropriate settings.
• Adhere to all safety, infection control, and quality standards set by regulatory agencies and the organization.
