Trumen Physicians + Associates is seeking a dedicated and experienced Physician to join our growing post-acute care team in Northwest Houston, TX. Independent Contractor (1099) position – Enjoy autonomy and tax flexibility
As a Post-Acute Care Physician, you’ll manage a panel of patients in skilled nursing and rehabilitation facilities. You’ll play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.
Provide in-person medical care to patients in assigned post-acute facilities
Perform assessments, manage treatment plans, and update documentation in the EMR
Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care
Participate in Trumen’s value-based care and transitional care programs
Develop strong working relationships with facility staff and leadership
Serve as a liaison between Trumen and facility teams
Be available in assigned facilities anytime during regular business hours on scheduled days
MD or DO with a current, unrestricted Texas medical license
Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
1+ year of experience in post-acute, geriatric, or SNF care preferred
Comfortable practicing independently in skilled nursing facilities
Strong clinical judgment, communication skills, and a patient-centered mindset
Reliable transportation within the Northwest Houston area
