This full-time role offers a base starting salary of $125,000, a comprehensive benefits package, paid malpractice, and opportunities for CME stipends and loan forgiveness as you contribute to high-quality, coordinated care across our regional network.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant with current NP/PA license.
• Post-acute care experience preferred.
• Strong communication and care coordination skills.

Responsibilities

• Manage transitions from hospital to post-acute settings with a focus on recovery.
