Job summary

Spectrum Medical Partners seeks a Post-Acute Nurse Practitioner (APRN) to deliver comprehensive care in skilled nursing facilities. The APRN will lead facility rounds, perform patient assessments, diagnose and treat conditions, and coordinate care with physicians and interdisciplinary teams.

Pulmonary experience is preferred but not required. This role emphasizes high-quality, patient-centered care, preventive education, and tests/treatments within a collaborative post-acute setting in

Qualifications

• Current Florida APRN license in good standing.
• National Board Certification.
• Active DEA registration.

Responsibilities

• Examine patients and obtain comprehensive medical histories.
• Perform routine facility rounds and patient assessments.
• Adhere to proper charting and documentation protocols.
• Collaborate with physicians and healthcare teams to develop and implement care plans.
• Order and interpret diagnostic tests and imaging.
• Analyze test results, diagnose conditions, and communicate findings to patients and staff.
• Prescribe medications and administer treatments as appropriate.
• Submit patient care plans and goals for periodic physician review.
• Promote preventive care and provide patient education on disease management, hygiene, and nutrition.
• Administer immunizations and vaccinations as required.
• Refer patients to specialists when necessary.
• Report deaths and contagious diseases to appropriate governmental agencies.
• Perform other duties as assigned.
