Spectrum Medical Partners seeks a Post-Acute Nurse Practitioner (APRN) to provide comprehensive medical care in assisted living facilities in Leesburg, FL. You will conduct rounds, assess patients, coordinate care with physicians, and manage follow-up plans to ensure high-quality post-acute care.

The role requires Florida APRN licensure, National Board Certification, DEA registration, and at least 1 year of post-acute or skilled nursing experience. Bilingual English/Spanish is a plus.
